We all have a role to play in recognising, removing, and assisting players who sustain a concussion to recover and return to rugby.

Reporting a suspected concussion is a crucial step in prioritising player welfare and ensuring appropriate care and follow-up. This form enables clear communication between coaches, managers, players, parents, Provincial Unions, schools/clubs, and medical professionals.

Exercising caution

Concussions aren’t always visible. By completing the Suspected Concussion Notification Form, you’re taking a practical step to protect the player’s wellbeing.

Trial notice: This form is currently being piloted with select Provincial Unions. If you’re outside these regions, please choose OTHER in the Provincial Union dropdown.

What to expect after you've sent in a concussion form:

The form will prompt you for key details about the player, the incident, and any observed signs or symptoms. Please be as accurate and specific as possible to support appropriate care.

After you submit, New Zealand Rugby will:

  • Provide support related to return to play and medical appointments.
  • Email the player, Provincial Union, and school/club to outline the suspected-concussion notification and the return-to-play process.
