Reporting a suspected concussion is a crucial step in prioritising player welfare and ensuring appropriate care and follow-up. This form enables clear communication between coaches, managers, players, parents, Provincial Unions, schools/clubs, and medical professionals.

By completing the Concussion Notification Form, you’re taking a practical step to protect the player’s wellbeing.

 

What Happens Next?

The form will prompt you for key details about the player, the incident, and any observed signs or symptoms. Please be as accurate and specific as possible to support appropriate care.

After you submit, New Zealand Rugby will provide:

  • Concussion education and guidance.
  • Information on returning to work/school and play.
  • An email to the player, parent, school/club and Provincial Union to outline the suspected-concussion notification.
